This photograph depicts a close left lateral view of a patient’s penis, revealing the presence of a cutaneous lesion of roughly 40-days duration, which was determined to be granuloma inguinale, also referred to as genital ulcerative disease, or Donovanosis. One month before, this patient’s symptoms were treated with a primary syphilis therapy. Initially, the size of the lesion decreased, but then began to increase, causing the patient to return to the VD Clinic, whereupon, a tissue smear tested positive for Donovan bodies. Granuloma inguinale is a sexually transmitted disease. It is a slowly progressive ulcerative condition of the skin and lymphatics of the genital, and perianal area, caused by the Gram-negative bacterium, Klebsiella granulomatis formerly Calymmatobacterium granulomatis.
