This 1969 image depicts what was an otherwise healthy child who’d developed extensive vaccinial scarring after contracting the virus, while still in-utero. Intrauterine infection of a fetus, referred to as fetal vaccinia, can occur following primary smallpox vaccination of a pregnant woman in the 2nd, or 3rd trimester from hematogenous spread of the virus to the amniotic fluid, or directly to the fetus.
