Known to be infected with the lepromatous form of leprosy, also known as Hansen’s disease (HD), this patient had been given a 0.1ml, subcutaneous injection of the antigen, lepromin, consisting of inactivated Mycobacterium leprae, i.e., leprosy bacteria, producing no reaction to the antigen. A positive skin reaction may be seen in patients with specific forms of leprosy, such as tuberculoid, and borderline tuberculoid leprosy. However, patients with lepromatous leprosy will not have a positive skin reaction. Lepromatous (Virchowian) HD, the most severe and transmissible form of the disease, produces no skin reaction to the antigen, lepromin. This is because an effective immune response to the bacterium is a result of a cellular immune response (T-cell mediated), rather than a humoral response (B-cell/antibody).
