This male patient presented with a cutaneous lesion located on the glans penis, distorting its normal contours, and causing severe edema, or swelling, of the penile shaft. The infection involved the bilateral inguinal lymph nodes, which had erupted at the surface of his skin. The infection was diagnosed as a case of granuloma inguinale, or Donovanosis, a sexually transmitted disease caused by the bacterium, Klebsiella granulomatis, formerly known as Calymmatobacterium granulomatis, and even earlier Donovania granulomatis. The causative organism is difficult to culture, and diagnosis requires visualization of dark-staining Donovan bodies on tissue crush preparation from a biopsy.
