This young Indian woman presented with a case of borderline, or dimorphous leprosy, with bilateral involvement of the patient’s buccinators, or cheek muscles, as well as dermatomyositis, evidenced by the cutaneous, rash-like discoloration overlying the muscles beneath. Leprosy is caused by the bacterium, Mycobacterium leprae. See PHIL 15504, for another view of this patient’s face. Borderline leprosy is the most common form of the disease, and when compared to tuberculoid, or lepromatous forms, is of intermediate severity. The skin lesions seem to be of the tuberculoid type, but are more numerous, sometimes affecting an entire limb. Peripheral nerves are affected as well, with ensuing weakness and anesthesia.
