This 22-year-old female Cardiff, Wales, smallpox patient presented with what was characterized as a malignant, semi-confluent, variola major rash, on the fifth day of focal eruption. Present, were confluent indurated lesions, and a delay in maturation with no vesicles. Note the hemorrhagic papule present on her left breast. This patient died on the tenth day post onset.
